The practice is open for new patients who live in the southern part or centre of Haarlem, postal code areas 2011, 2012 (with the exception of the postal codes 2012 XK and 2012 WA to WZ), 2013, 2014, 2015 and 2019 ("Raaks). Please contact our assistant if you need more detailed information

To register at our practice, please bring your filled out subscription form, your ID (passport or EU-identity card), your Burger Service Nummer (= the Dutch social security number) and your medical insurance papers to our assistant.
Temporary residents/Expats
Visitors or temporary residents/Expats in our area in need of medical advice, can call our assistant to make an appointment with one of the doctors.
